Palo Alto, Calif. & Cincinnati, Ohio - September 19, 2017 - Temasys Communications today announced that RxConcile has chosen the Temasys real-time communications platform to power its virtual pharmacist app aimed at reducing patient readmissions through real-time medication counseling.
The RxConcile solution enables pharmacists to connect online via live video and voice chat with discharged long-term care patients and caregivers via a web browser in a fully secure, HIPAA-compliant environment. The pharmacist drives the video conference where details of the patient’s care and medication details can be fully explained and recorded for secure playback. via the HIPAA-compliant patient portal.
“Medication misadventures not only put patient lives in jeopardy but cost the healthcare industry billions of dollars every year in preventable hospital readmissions,” said Chad Worz Pharm.D and CEO at RxConcile. “The Temasys technology is a critical part of our application, which enables us to do live video and audio with the pharmacist and most vulnerable patient segments in the healthcare system, safeguarding lives and addressing this critical area of need.”
Medication Mismanagement Top Reason for Medication-Related Readmission
Over 24 million patients requiring some degree of medication counseling are discharged from hospitals each year while nearly 8 Million patients are discharged to post-acute care facilities per year. The national average for hospital readmissions that occur post-discharge from either a hospital or post-acute care setting is more than 23 percent. This means that nearly one out of every four recently discharged patients will return to an institutional care setting following an unsuccessful release to home. Studies suggest that 66 percent of the time, the patient’s readmission is the result of some form of medication mismanagement. In addition, a study by retail pharmacy CVS suggests that 50 percent of those medication-related readmissions could be avoided with proper medication counseling and reconciliation.
Temays Provides Secure Platform for Online Communication
The Temasys Platform is embedded within the RxConcile application and provides teleconference technology that allows a secure conversation between the pharmacist, the patient, and their caregiver. Temasys enables a fully encrypted exchange of audio, video, data and chat streams in a secure, stable and reliable manner between all connected parties.
In addition, to live video and voice chat, Temasys helps to streamline the complex medication instructions given to patients by recording the conversations, which are securely accessible via the RxConcile patient portal.
“This is a great example of the trend in ‘communication-core’ applications,” said Bent Rye, Temasys CEO. “We aim to help our customers deliver end-to-end engagement solutions, powered by embedded real-time communication. We’re very excited that our platform can play a key part in RxConcile’s larger effort to improve patient care.”
